Is there any open source or proprietary solution that helps scanning documents, evaluate them and store the results in a database?

  1. Scanning: Scan documents into an image or pdf file. The documents I currently need to process have handwritten parts too, but those are handled manually by the employees. I need to read a document number, which is printed and process a bunch of checkboxes (which are checked by hand).
  2. Evaluate: The read data should be display to a user for a quick check and he should be able to fix the errors.
  3. Database: I need to store the data in a database in the end (not the image itself). I could use a solution where the program could export a selection of data to a file which I could process later (CSV maybe?).

Main question is which checkboxes got marked by the user.
